# Virginia RHTP Mobile & Hybrid Care RFA — Central Shenandoah Planning District Commission (Roanoke-Alleghany/Central Shenandoah/New River Valley Collaborative)

Competitive RFA under the Virginia Rural Health Transformation Program — Mobile & Hybrid Care sub-initiative, led by the Central Shenandoah Planning District Commission in collaboration with the Roanoke-Alleghany, Central Shenandoah, and New River Valley Regional PDCs. This opportunity is for projects that develop and deploy mobile and hybrid care models to serve rural residents within the specified service area. Eligible models include mobile care units, telehealth kiosks, 'clinic in a box' models, provider-to-provider electronic consultations, and hub-and-spoke care centers supported by technology enablers. ## Eligibility

Eligible applicants must be physically located in, or provide direct services to, rural Virginia communities as defined by the Virginia RHTP (using the FORHP baseline definition). Eligible projects must serve residents in the following counties: Alleghany, Bath, Craig, Floyd, Franklin, Giles, Highland, Pulaski, and Rockbridge; as well as the cities of Buena Vista, Covington, Lexington, and Radford.

## Requirements

Applications must include a completed application form, project narrative and work plan, detailed budget and budget narrative, letters of commitment from each core partner, and required organizational documentation as specified in Section 4.2 of the RFA. Projects must align clearly with the eligible approaches and target populations listed in the RFA. Applicants must agree to report on standard performance measures and participate in program evaluation activities as outlined in Section 5.4.
