## Eligibility

## Requirements

Provide organizational readiness assessments; conduct EHR/EMR modernization; deliver interoperability, data quality, and analytics support; offer technical assistance; coordinate implementation; and administer Rural Value-Based Innovation funding as outlined in the solicitation description. Respondents must follow applicable federal grant requirements and the procurement process per Sub-chapter 4.5, HAR.
