Requirements Checklist

• Required submissions include a project narrative (with sections on need, project description and alignment with one or more RHTP SHIFT programs, partnerships, baseline data, sustainability plan), a detailed work plan and timeline (using the provided template), a budget and justification, a performance metrics and evaluation plan, an organizational chart, and combined resumes/CVs of key project personnel. Conditional required documents include audited financial statements, IRS determination letters, Board of Directors lists, and letters of support or MOUs from key partners as applicable.
