# New Hampshire RHTP: EMS Grant Opportunity Through the Foundation for Healthy Communities

Grants up to $25,000 to stabilize or strengthen existing rural EMS systems, $100,000–$400,000 for expansion of existing Mobile Integrated Health (MIH) programs, and $300,000–$2,000,000 for new/expanded MIH programs and regional partnerships in New Hampshire. Funding supports technology upgrades, workforce training, care coordination, and sustainable transformation of rural emergency and mobile health care. The Foundation for Healthy Communities (FHC) manages the program under GO-NORTH, with awards made to eligible nonprofit, for-profit, and community-based EMS and health organizations, including those pursuing EMS licensure. Eligible activities include technology investments, continuing education, regional partnerships, MIH service delivery, staff upskilling, startup costs, and developing reimbursement model demonstrations. Each funding component requires a separate application. Multiple eligible organizations may collaborate, with one designated lead. Initial awards run through September 30, 2027; future funding is contingent on performance and GO-NORTH authorization. Allowable costs include personnel, technology, regional collaboration, and associated MIH startup/operational costs. Equipment purchases >$10,000 require prior written approval. Ineligible entities are individuals or organizations with outstanding legal proceedings involving NH or government entities. Ineligible activities include capital expenditures for facilities, duplicative clinical services, research projects, lobbying, and direct cash workforce recruitment/retention benefits. Applications must detail organizational capacity, alignment with GO-NORTH RHT initiatives, sustainability plan, partnership/collaboration plan, and include required attachments (work plan, budget, letters of support, IRS status/certificate for nonprofits).
