## Eligibility

Open to eligible U.S.-based and U.S.-owned entities. Preference is given to Nebraska-based companies. Applicants must have a functional prototype at Technology Readiness Level 4 or higher, and funded projects must be piloted or deployed at rural Nebraska sites.

## Requirements

Applications must address rural health needs, benefit Medicaid, low-income, or vulnerable rural consumers, focus on chronic disease prevention or management, demonstrate an unmet need, and improve quality, affordability, or access to care. Cohort 1 funds and milestones must be completed by September 30, 2027.
