the Rural Health Transformation Program. Awards support HomVEE-approved evidence-based home visiting models only. Up to $100,000 available for model licensing fees/staff training for organizations starting a new home visiting program, or up to $488,000 for organizations expanding an existing evidence-based program. Funds are expansion-only — may not supplant existing services. Grants are renewable for up to 4 additional years subject to performance and funding availability.
Eligible applicants are Missouri-based nonprofit organizations or local public agencies that plan to establish or expand a HomVEE-approved, evidence-based, nurse-delivered home visiting program serving rural populations. Applicants must demonstrate capacity to deliver the proposed home visiting model and agree to required data collection and reporting.
