# Michigan Rural Health Transportation Food Club - 2027

Synopsis As part of RHTP’s Care Closer to Home Blueprint Initiative, the Improving Care Delivery & Access through Transportation Blueprint Grant Program is designed to transform how rural residents access care by creating sustainable, community-based pathways that better serve people where they live and work. These efforts will align partners across health and social sectors to improve care delivery, expand transportation access, and strengthen access to critical services. By promoting innovation in rural areas that lack robust transit options, the transportation blueprint aims to make health care more efficient, coordinated, and person-centered
