# Michigan Rural Health Partnership Chronic Disease Michigan Hubs - 2027

Synopsis As part of Rural Health Transformation Program's Transforming Rural Health through Partnerships Initiative, the Chronic Disease Collaborative Care Fund supports Michigan’s rural health providers and community organizations in advancing innovative, collaborative approaches to chronic disease prevention and management. This funding opportunity is designed to pilot and scale evidence-based interventions that improve outcomes for diabetes, hypertension, and other chronic conditions, while leveraging existing rural health and state networks and strengthening cross sector partnerships. Integration of community health workers and behavioral health supports are included as complementary strategies to enhance care delivery.
