As part of RHTP's Care Closer to Home Blueprint Initiative, the Advancing Rural Behavioral Health Blueprint aims to transform access to care in rural communities by creating sustainable, community-based pathways that bring health services closer to where people live and work. By connecting partners across health and social sectors, the initiative reimagines care delivery, improves transportation access, and simplifies access to primary and behavioral health. Through innovative approaches to chronic disease management, maternal and child health, and behavioral health, it seeks to improve outcomes while making care more efficient, coordinated, and person-centered. This funding opportunity supports innovative adaptations of traditional care models to expand access to community-based behavioral health services in rural areas while reducing costs by avoiding unnecessary hospitalizations and limiting reliance on law enforcement.
Eligibility
Eligible applicants include nonprofit organizations, local public health departments, federally qualified health centers, rural health clinics, Critical Access Hospitals, tribal health organizations, and other public or private entities with demonstrated experience in behavioral health or rural health services within Michigan.

Requirements Checklist
Applicants must submit a completed application through EGrAMS, including a project narrative, detailed budget and budget narrative, organizational chart, proof of eligibility, letters of support or partnership (where relevant), and required federal and state certifications. All applications must address sustainable community-based service delivery, partnership development, and measurable outcomes aligned with RHTP goals.
