Iowa Mobile Integrated Health Projects (RFP PHTHOET27001)

Summary

Notice of intent to release RFP PHTHOET27001 for up to seven EMS-led Mobile Integrated Health projects to expand preventive, chronic disease, prenatal/postpartum, and acute in-home care in rural Iowa. Projects equip EMS agencies with training, telehealth, and clinical tools needed for coordinated home care aimed at reducing avoidable ED utilization and improving local healthcare access.

Eligibility

Anticipated to be open to EMS agencies and organizations capable of providing community-based healthcare services in rural Iowa. See full RFP upon release for definitive eligibility.

Requirements Checklist
