## Eligibility

Eligible applicants are Illinois state universities, community colleges, and rural hospitals, as specified in the Illinois Rural Health Transformation Program (RHTP) Initiative 3.A. Sub-awards are allocated according to named entities/formulas; not a traditional open competition.

## Requirements

Recipients must use funds for workforce incentive, recruitment, and clinical training program expansion in alignment with Initiative 3.A. Annual reporting on program metrics and workforce outcomes is required according to CMS/HFS guidelines. Funds must be obligated by 10/30/2026 for Y1.
