## Eligibility

Eligible applicants are rural hospitals and hospital networks in Illinois. Sub-awards may be directed to Illinois Critical Access Hospital Network (ICAHN) as specified in budget allocations. Only entities meeting CMS rural designation and state requirements may apply.

## Requirements

Funds must be used for chronic disease prevention programs. Grant recipients must obligate funding before October 30, 2026 (Year 1 period end). Award reporting and participation in the Rural Health Transformation Program required.
