# Healthy Hometowns: Mobile Integrated Health (RFP # PHTHOET27001)

The Iowa Department of Health and Human Services, through RFP #PHTHOET27001, seeks applications to initiate or expand Mobile Integrated Health (MIH) services within rural Iowa. The program supports EMS agencies or community-based clinical providers in launching or broadening treat-in-place care, telehealth-enabled assessments, chronic disease management, post-discharge follow-up, prenatal and postpartum home-based support, medication reconciliation, and other in-home interventions. Applicants may propose to initiate a new MIH program or expand an existing MIH program to additional rural counties. The goal is to improve health outcomes for rural Iowans, reduce avoidable emergency department and inpatient utilization, and strengthen care capacity through coordinated, community-based services. This federally funded initiative, part of the CMS Rural Health Transformation Program (CMS-RHT-26-001), prioritizes projects that cover multiple rural counties and leverage telehealth and care coordination tools.
