# Healthy Eating, Active Recreation, and Transformation (HEART)

Supports rural Arkansas prevention and wellness projects across six HEART sub-initiatives: GROW Kids, FARM, MOVE, FAITH, HEAL, and IMPACT. HEART focuses on coordinated, community-driven strategies targeting nutrition, physical activity, and chronic disease prevention, integrating schools, faith organizations, health providers, and community partners. The initiative aims to expand preventive care, promote healthy lifestyles, reduce chronic disease, improve behavioral health outcomes, strengthen workforce capacity, and support data-driven monitoring. The program encourages diverse applicants to propose measurable, scalable models that strengthen rural health systems and address underlying health disparities.
