Arizona Center for Rural Health (AzCRH)
The Arizona Center for Rural Health (AzCRH) provides a comprehensive toolkit to assist applicants preparing submissions for the Arizona RHTP. Resources include application templates, project and budget narrative examples, policy/program briefs, eligibility and compliance checklists, technical assistance guides, and data analysis tools. The toolkit supports rural hospitals, providers, clinics, and stakeholders responding to statewide RHT funding and policy opportunities led by the Governor’s Office and AHCCCS.
Intended for Arizona rural hospitals, Critical Access Hospitals, Rural Health Clinics, Federally Qualified Health Centers, tribal health organizations, and health providers eligible under the definitions and requirements of the Rural Health Transformation Program (RHTP) as detailed in Arizona's RHTP Plan and the One Big Beautiful Bill Act (H.R. 1, Title VII, Sec. 71401, 2025).
