Team-Based, Value-Driven Primary Care Transformation in Interior Alaska — Foundation Health Partners proposes to transform the primary care system in Interior Alaska from an overburdened physician-centric fragmented, and reactive model to a patient-centered, coordinated, team based, technology enabled system that delivers timely, preventive, and continuous care for rural and frontier residents. The project will develop the clinic, IT, data and workforce infrastructure, and core competencies necessary for long-term participation in value-based care arrangements.

PeaceHealth Ketchikan Medical Center

Rural Surgical Transformation: Expanding Access and Modernizing Care — PeaceHealth Ketchikan Medical Center proposes introducing the first robotic assisted minimally invasive surgeries in orthopedic, gastroenterology, urology, gynecology, and general surgery to Southern Southeast Alaska. This evidence-supported solution will expand local specialty capacity, reduce out-of-area referrals, and improve outcomes by overcoming longstanding access barriers. By enabling high-quality procedures close to home, the project transforms healthcare in the region and works to reduce health disparities.

Envoy Integrated Health

Statewide Population Health Service Organization — Envoy Integrated Health proposes building a statewide shared services model that gives Alaska’s providers access to high quality and affordable electronic health records, data analytics, and care coordination tools. This statewide population health services model addresses Alaska’s fragmented healthcare system, limited interoperability, and high infrastructure costs by enabling Alaska’s providers – especially those in small, independent, or rural practices – to manage the health of their patients and participate in value-based care.

Southcentral Foundation

Yeshjesh T'uh Crisis Project — Southcentral Foundation’s Yeshjesh T’uh Crisis Project proposes strengthening system capacity by providing new innovative services, connecting the gaps in fragmented systems, and increasing access to culturally competent mental health and substance use disorder services in Mental Health Professional Shortage Areas here in Alaska. The proposed Yeshjesh T’uh Crisis Project provides high-intensity behavioral health services, including crisis stabilization and crisis residential care, for individuals from rural and remote Alaska communities.

Ketchikan Indian Community

Rural Health Through Partnerships - Salmon Falls Healing Center — Ketchikan Indian Community (KIC) will transform access to behavioral health services by developing and operationalizing the Salmon Falls Healing and Wellness Center, a first-of-its-kind tribally led residential treatment and recovery campus in Southeast Alaska. The project will expand access to coordinated, culturally responsive care by integrating residential treatment, behavioral health services, recovery supports, care coordination, workforce development, and community-based healing services into a single continuum of care.

Norton Sound Health Corporation

New Computers/Servers for Norton Sound Health Corporation — Norton Sound Health Corporation proposes installing modern core data storage, high-speed routing networks, and clinical workstations to replace end-of-life IT infrastructure that threatens system reliability and patient data security across fifteen remote village clinics and the regional hub of Nome.

Alaska Native Tribal Health Consortium

Statewide Tribal Pharmacy Development — Alaska Native Tribal Health Consortium proposes expanding access to medications and pharmacist-led care through medication access and optimization services, pharmacy automation, modernized pharmacy systems, and statewide pharmacy workforce development to address barriers to medication access, care coordination, and pharmacy service capacity for Alaska Native people and rural communities across Alaska.

healthEconnect Alaska

Statewide Health Information Exchange Expansion — healthEconnect Alaska proposes to strengthen Alaska’s statewide health data infrastructure by expanding health information exchange connectivity and interoperability, particularly for small and rural healthcare providers. Key activities include supporting EMR interoperability standards, providing technical assistance and connectivity funding, expanding population health dashboards and claims-based analytics to support Pay For Value projects, and strengthening care coordination through real-time event notifications, care team visibility, and referral tools. These activities align with milestones identified in DOH’s RHTP application including 1) Expand statewide health information exchange; 2) Enter contracts to expand HIE interoperability; and 3) Provide targeted investments for participating in electronic health record systems.

Providence Health & Services- Washington

Coordinating to Drive Redesign and Sustainability of Alaska's Graduate Medical Education Programs — Providence Health & Services -Washington proposes to strengthen workforce capacity by stabilizing and enhancing the Alaska Family Medicine Residency Program. This project invests in recruitment, rural training, faculty development, and clinical training infrastructure to address Alaska’s shortage of primary care physicians and limited in-state residency capacity. This work supports a critical physician pipeline serving rural, frontier, and geographically dispersed communities across Alaska.

Department of Education & Early Development

Alaska School District Healthcare Programs of Study — The Alaska Department of Education & Early Development (DEED) will provide competitive subgrants to school districts, partnering vocation training institutions and local regional employers to deliver programs of study in healthcare careers to secondary students. Public school districts currently receive federal Carl Perkins funding to provide exposure and training to students to connect them to workforce opportunities after graduation. Of Alaska’s 53 districts, 36 small and rural districts receive only $20,000 of these federal funds to provide Career and Technical Education across fourteen different career clusters of which Healthcare and Human Services is just one. Currently, 26 of Alaska’s smallest districts do not offer any federally funded healthcare career and technical education due to limited capacity or resources. Each school district is part of DEED’s existing network that supports and promotes career and technical education programming. The healthcare program of study grants would allow smaller rural districts to focus on providing options to rural students to explore healthcare career options and receive technical training leading to careers in the local, regional, and statewide healthcare sector. The grants would serve students with priority given to smaller rural districts facing economy of scale limitations.

Ryan Air

Using Advanced Air Mobility Systems to Deliver Perishable Pharmaceuticals to Remote Destinations — Ryan Air proposes to deploy autonomous cargo drones and electric vertical take-off and landing (EVTOL) aircraft to deliver pharmaceuticals and transport patients and healthcare providers across roadless communities in the Bethel and Dillingham regions. This project addresses the near-total lack of reliable, affordable pharmaceutical access and medical transport in remote Alaska Native villages, where geography, weather, and the absence of road infrastructure create life-threatening gaps in basic healthcare.

Department of Corrections

Strengthening Alaska’s Correctional Health System Through Virtual Care — The Department of Corrections (DOC) proposes to implement telemedicine and teledental equipment across correctional facilities. This expansion connects incarcerated Alaskans with healthcare providers in their home communities and across the state, improving continuity of care both during incarceration and after release. Many individuals enter custody with untreated chronic medical and mental health conditions; by introducing them to telehealth services and establishing relationships with community-based providers, DOC supports smoother transitions to care upon reentry.

Kodiak Island Borough

Kodiak Island Hospital Chiller Replacement to Ensure Access to Care — Kodiak Island Borough, owner of the hospital facility, proposes a capital expenditure and infrastructure project that will ensure access to care by replacing the facility's chiller system, a critical infrastructure element supporting the environment of care and sterilization process used by the Emergency, Surgical, and Obstetrics departments. The existing chiller is functioning at ~35% capacity, limiting the care that can be provided on the island. This project will improve reliable access to medical care for residents, seasonal workers, and visitors in the Kodiak Island Borough.

Eastern Aleutian Tribes

Healthier Alaskans: Rural Oral Health Access and Dental Infrastructure Modernization Initiative — Eastern Aleutian Tribes proposes to modernize regional dental infrastructure, replace aging equipment, deploy portable dental treatment systems, and expand oral health access across remote communities. The project addresses limited access to dental care, aging clinical infrastructure, and geographic barriers affecting residents of Adak, Akutan, Cold Bay, False Pass, King Cove, Nelson Lagoon, and Sand Point.

Ophthalmic Associates

MyEyesAI Hub-and-Spoke Tele-Ophthalmology for Rural Alaska. — Ophthalmic Associates proposes to establish a hub-and-spoke teleophthalmology network using MyEyesAI to support standardized eye intake, image capture, specialist review, triage, and referral coordination, thereby addressing delayed access to ophthalmology and avoidable travel for rural, remote, and frontier Alaskans across participating clinics, optometry practices, and community health sites.

Juneau Economic Development Council

St. Ann’s Center for Integrated Care – Redevelopment Project — Juneau Economic Development Council (JEDC) proposes to redevelop St. Ann’s/Crimont Center in Juneau, Alaska into an integrated nonprofit care center with several regional partners to address gaps in senior housing, transitional housing, community geriatric care, behavioral health, home- and community-based services, congregate meals, care navigation, and workforce development to serve vulnerable seniors and adults with developmental, cognitive, behavioral health, and related needs.

Southcentral Foundation

Expansion of Specialty Pediatric Services — Southcentral Foundation proposes to expand its Specialty Pediatric Clinic, a critical statewide access point that addresses gaps in specialized care for Alaska Native and American Indian children with complex medical and developmental needs across Alaska, including those from rural, remote, and frontier communities who must travel for care.

Alaska Stroke Coalition

Statewide Deployment of Clinical AI Decision Support Network Across Alaska Hospitals — Alaska Stroke Coalition proposes a statewide AI imaging network across 21 acute care hospitals that pairs broad multi-pathology triage with advanced stroke imaging, so every site — urban or rural — receives the same rapid interpretation support for time-critical findings: stroke, hemorrhage, pulmonary embolism, aortic and spinal injury. Utilizing two complementary, cloud-based platforms that run on existing PACS without new imaging hardware — diagnostic breadth everywhere, plus the stroke depth needed to treat and transfer faster, while surfacing incidental findings for earlier intervention.

Municipality of Anchorage

Municipality of Anchorage: Connecting Crisis Care and Safety Net System to Rural Healthcare — Municipality of Anchorage (MOA) proposes to implement technology and test pilot approaches to improve outcomes from behavioral health crisis response, especially for rural and Tribal enrollees seeking care in Anchorage. The MOA will implement interoperable client records to coordinate care with local, Tribal, and other healthcare providers, sustain services, and capture data. MOA will implement and evaluate pilot projects to fill critical gaps in Anchorage's Crisis Care and Safety Net System to improve patient outcomes and system efficiency, and identify possible new Medicaid waiver services.

Office of Children's Services, Department of Family & Community Services

Alaska Comprehensive Child Welfare Information System (CCWIS) Project — The Office of Children's Services proposes to modernize the Comprehensive Child Welfare Information System (CCWIS) . CCWIS modernization is especially consequential for Alaska’s rural and remote communities, where families are served across vast distances, limited connectivity, and a dispersed network of providers, regional hubs, and tribal partners. This investment is designed to provide lasting value beyond the initial RHTP performance period by advancing a modular CCWIS that integrates with existing State of Alaska systems and is built to federal design and interoperability standards.

Yukon-Kuskokwim Health Corporation

Modernizing Village Clinic Network Infrastructure to Support Telehealth and Cloud Systems — Yukon-Kuskokwim Health Corporation proposes expanding its network infrastructure and capabilities to strengthen network connectivity and cybersecurity protection and enable more sophisticated technology platforms. This project will span across 48 village clinics, addressing aging systems that limit reliable connectivity, cybersecurity, telehealth, and cloud-based clinical tools in the Yukon-Kuskokwim Delta.

Moda

Advancing Value-based Payment Readiness through PCMH Implementation on the Kenai Peninsula — Moda proposes to partner with Central Peninsula Hospital and South Peninsula Hospital to support implementation of the National Committee for Quality Assurance’s (NCQA) Patient-Centered Medical Home (PCMH) model across four clinic sites. This project will help strengthen partners’ quality improvement and reporting infrastructure, support expanded access to care for rural populations and increase partners' readiness to participate in advanced value-based payment arrangements in preparation for Alaska’s planned 1115 Medicaid coordinated care demonstration project on the Kenai Peninsula.

Yukon-Kuskokwim Health Corporation

Expanding Community Health Aide Training Capacity in the YK Region — Yukon-Kuskokwim Health Corporation proposes expanding the Community Health Aide Training (CHAT) program in Bethel to increase programs capacity and expand the regional healthcare workforce. The Community Health Aide Training Center in Bethel trains most of the Community Health Aides serving the Yukon-Kuskokwim Delta and currently has a waitlist of 36-40 trainees for session one. Once trained, these community health aides provide routine, acute, and emergency direct patient care in village clinics.

Fond of You Inc (Somethings)

Virtual Low-Bandwidth Certified Peer Support for Youth and Young Adults in Rural Alaska — Somethings will provide accessible, low-bandwidth virtual peer mentorship to over 3,000 Alaska youth ages 13 to 26 in rural, frontier, and Alaska Native communities. Through Alaska-based Certified Peer Specialists who share lived experience and local context, the program expands behavioral health access, strengthens the peer support workforce, and opens a new front door for youth mental health care, with local partners across the state for referral pathways. The goal is to reduce depression, anxiety, and emergency department utilization in areas with limited behavioral health access.

Haines Borough

Haines Wellness Initiative — Haines Borough proposes establishing a coordinated community wellness initiative that strengthens behavioral health services, improves crisis response resources, expands therapeutic aquatic and recreational programming, and increases senior support services. The project addresses mental health challenges, suicide risk, social isolation, limited behavioral health resources, and barriers to care by bringing together healthcare providers, community organizations, local government, and residents to improve access to wellness opportunities for people of all ages in Haines Borough.

Tanana Chiefs Conference

Indigenous Healing Integration & Rural Wellness Transformation Initiative — Tanana Chiefs Conference proposes a permanent Department of Indigenous Health and Traditional Medicines that integrates Indigenous healing into primary care, behavioral health, chronic disease prevention, and telehealth. The project builds a clinical apprenticeship pathway for Alaska Native providers to learn applications of hands-on healing, including plants, diet, and lifestyle approaches to wellbeing and addresses the shortage of culturally safe and comprehensive Indigenous healing services for Alaska Native people in Fairbanks and 27 rural Interior Alaska communities served by TCC.

True North Recovery, Inc.

Wasilla Peer Crisis Residential Respite Center — True North Recovery proposes the Wasilla Peer Crisis Residential Respite Center to bridge a critical gap in the regional crisis continuum. Facing 45 opioid-related fatalities and responding to over 1200 mobile crisis calls, local data confirms an urgent need for subacute residential care. By expanding crisis services and leveraging current partnerships, this project offers a replicable, data-driven model for Alaska. This pilot establishes an actuarially sound, peer-led framework for sustainable, value-based care, that can be replicated across Alaska.

Alaska Association on Developmental Disabilities

Strengthening Alaska's Rural Disability Workforce and Support System — Alaska Association on Developmental Disabilities proposes expanding GoodLife Innovation’s evidence-based workforce stabilization, technology advancements, remote support, and education surrounding innovating housing models, including host homes, to strengthen access to consistent services, increase workforce stability, and improve continuity of care for individuals with disabilities and aging adults in rural and remote Alaska.

Statewide Independent Living Council

Community Partners Improving Home Accessibility — Statewide Independent Living Council of Alaska proposes to install durable, ADA-compliant modular aluminum and composite ramps at the homes of Alaskans with disabilities and aging Alaskans statewide, replacing wood ramps that fail within three to five years in Alaska's climate, for rural, remote, and frontier communities where existing home modification programs leave residents waiting years for safe entry access.

Chugachmiut

Expanding Rural Access to Comprehensive Behavioral Health Services — Chugachmiut proposes the development and implementation of a comprehensive program for the Chugach region and State of Alaska that provides psychological and neurological assessments, autism services, fetal alcohol spectrum disorder assessments, peri natal services, therapy services, and case management support, thereby expanding and localizing access to specialized behavioral health services for rural communities by providing telehealth and in-person clinical services, allowing families to receive timely evaluations, treatment planning, and follow-up care locally.

Alaska Child Welfare Referral Process Improvement Project — The Office of Children's Services (OCS) proposes investing in a bridge solution that works with Alaska's existing systems and the health information exchange to close gaps now, while remaining compatible with the future Comprehensive Child Welfare Information System (CCWIS).

Arctic Slope Native Association

Center for Supporting Healthy Families Project — The Arctic Slope Native Association proposes the expansion of health care services in the Arctic Slope Region through the creation of a single coordinated center for families to access health care referrals, health education, social services, and other family supports to address specific needs for their children. This project will address gaps in service delivery where referrals might get stuck or lost between programs, parents complete multiple or duplicate intake/eligibility forms, or people seeking services may discover that they are in the wrong building to access a particular program.

Arctic Slope Native Association

Surgical Suite Renovation — The Arctic Slope Native Association proposes to renovate space at the Samuel Simmonds Memorial Hospital to create a surgical suite in Utqiagvik Alaska. This project provides access to surgical care for approximately 10,000 residents that would otherwise have to leave the community. Creating this access reduces travel impact on expectant mothers, elderly/medically fragile patients, and improves response times for trauma patients.

Alaska Housing Finance Corporation

Healthcare Workforce Housing Catalyst — Alaska Housing Finance Corporation proposes offering flexible housing assistance for up to 150 healthcare workers in 5 communities with 10,000 or fewer people that are located off the road system. This assistance will be for healthcare workers new to the community and can be used for up to $1,000 / month rent or homeowner assistance for 12 months as a continually employed healthcare worker. The assistance can flex from rental to homeowner assistance during the service period and will help recruit and retain new healthcare workers struggling with housing availability and costs.

Central Peninsula General Hospital

Rural Healthcare Access Expansion Through Technology-Enabled Care — Central Peninsula Hospital proposes investments in digital health infrastructure, including clinical documentation improvement, telehealth expansion, and cybersecurity enhancements. Serving a geographically dispersed population across the Kenai Peninsula, CPH faces workforce shortages, increasing patient complexity, documentation burdens, and limited access to specialty care. This project will reduce administrative workload, improve care coordination and regulatory compliance, strengthen reimbursement and value-based care participation, and expand access to high-quality local health services.

Mountain View Health Services

Mountain View Community Health Transformation and Sustainability Initiative - Building a Neighborhood Health System Without Walls — Mountain View Health Services proposes a Physician-led, Nurse-enabled, Social Work-integrated, Early Life Community Health Worker (EL-CHW)-supported neighborhood health system "without walls". The project combines integrated primary care, behavioral health, substance use treatment, telehealth, home visiting, and mobile health with a new Medicaid-supported EL-CHW model providing prevention and care coordination from preconception through age five to improve access and outcomes for underserved residents of Anchorage.

City of Nenana

Nenana Healthcare Alliance — City of Nenana proposes renovating existing indoor and outdoor community space to create a year-round location for community gathering, physical activity, hygiene access, showers and water filling services for residents without running water, behavioral health and recovery programming, prevention activities, and health-related services coordinated through local partnerships and a Technical Assistance provider, addressing critical gaps in community infrastructure, social connection, and access to health-supporting resources for Nenana and surrounding rural residents.

Matanuska-Susitna Borough School District

Building the Mat-Su Healthcare Workforce: High School Pathways for Rural Alaska — Matanuska-Susitna Borough School District proposes building a regional healthcare workforce training network and certification hub, addressing healthcare workforce shortages, limited training capacity, and barriers to career entry for students and healthcare employers across the Mat-Su Borough and rural Alaska.

Alaska Commercial Company (AC Stores)

Building healthier communities through containerized aeroponic mini-farms off the road system — Alaska Commercial Company (AC Stores) proposes to grow fresh produce locally and year-round in four of the larger communities that we serve. We intend to use compact indoor growing systems to address the unreliable supply of fresh fruits and vegetables in remote communities across Western, Northern, and Southeast Alaska. All communities in this project are not on the road system.

Tlingit & Haida Foundation

Rooted in Wellness: Building a Sustainable Model for Tribal Behavioral Health — The Central Council of Tlingit & Haida Indian Tribes of Alaska, through its 501(c)3 Tlingit & Haida Foundation (Foundation) proposes to build a sustainable Tribal behavioral health system for Southeast Alaska by expanding access to culturally grounded behavioral health services, developing a Tribal provider workforce pipeline, and establishing the billing, credentialing, and health information infrastructure needed to address workforce shortages, unmet behavioral health needs, and long-term service sustainability for Tribal citizens in rural and remote communities.

Tanana Chiefs Conference

Strong Traditions, Healthy Communities: Lifestyle & Preventive Medicine Initiative — Tanana Chiefs Conference proposes an integrated Lifestyle Medicine and Clinical Prevention model to strengthen prevention and improve early detection. The project addresses high burdens of chronic disease and cancer, low screening rates, and barriers to care across the TCC region. Built from within TCC in response to community-identified needs, the initiative redesigns preventive care delivery and expands access to lifestyle medicine grounded in locally tailored, community-driven approaches that reflect rural health priorities focused on prevention, wellness, and healthy communities.

Alaska Native Tribal Health Consortium

Capacity and Bed Management to Improve Access to Care, Patient Flow, and Care Coordination — Alaska Native Tribal Health Consortium (ANTHC) proposes a health system-wide bed capacity management improvement initiative that integrates operational process redesign and staff training enabled by the Cerner Bed Capacity Management Module to improve patient flow, resource utilization, and care coordination, addressing capacity constraints and delays that affect rural and underserved populations traveling from remote Alaska communities to access specialized healthcare services and higher levels of care unavailable locally.

Alaska Association on Developmental Disabilities

The Road Home: Intensive Care and Community Integration for Alaskans with Complex Needs — The Alaska Association on Developmental Disabilities proposes to expand Alaska's capacity to support individuals with intensive needs by establishing a new level of care option for Alaskans with intellectual and/or developmental disabilities and other mental health disabilities whose behavior support needs cannot be met in standard community settings. This addresses a long-identified gap in Alaska's continuum of care by creating time-limited step-down settings with dedicated clinical oversight designed to stabilize behavior and support planned transition back to community-based living.

Alaska Jewish Campus

Alaska Friendship Circle All-Inclusive Early Childhood Play Center & Programming — The Alaska Jewish Campus proposes developing an inclusive, indoor early-childhood play center, enhancing the existing Outdoor Play Center and providing year-round warm, safe, accessible opportunities for play, movement, and social connection, addressing the shortage of inclusive recreation and developmental support opportunities for young children with disabilities and their families. Open to the public, free of charge, the Play Center serves children from Anchorage, Mat-Su and visitors from across Alaska.

Native Village of Tanana

Tanana Supportive Housing Program for Individuals Entering Recovery — The Native Village of Tanana, in partnership with the Tanana Chiefs Conference, proposes to establish a culturally grounded supportive housing program paired with outpatient clinical treatment and cultural connection, Elder guidance, and meaningful community roles for adults entering recovery from substance use, addressing the lack of transitional, step-down care between residential treatment and return to home community for rural Alaska Native people in the Interior region.

Southcentral Foundation

Rural Pharmacy Upgrade — Southcentral Foundation (SCF) proposes to improve pharmacy facilities and replace outdated secure, pharmacy-monitored, medicine-dispensing units (called “PickPoints”) in 14 rural clinics from Chitina to St. Paul, bringing SCF’s telepharmacies into functional alignment with security, software, and environmental needs, and making any minor modification necessary to ensure security, safety, and connectivity.

Medicaid Accountable Care Organization Pilot — Envoy Integrated Health proposes to pilot a Medicaid Accountable Care Organization (ACO) that brings providers together to coordinate care, improve quality, and reduce avoidable costs for Alaska Medicaid beneficiaries. Our project will take a tried-and-true value-based care model, which Envoy has already proven can be successful in Alaska, and translate it to Alaska’s Medicaid population.

Traditional Healing Project — Southcentral Foundation proposes to integrate traditional healing practices at its rural community health centers, standard clinical services, and the Alaska Native Medical Center (ANMC), by expanding the Traditional Healing practitioner base; creating an Alaska traditional healing network and exchange; and improving long-term sustainability. This project will have state-wide impact through ANMC services and addresses the need for traditional wellness practices, such as ceremony, ritual, emotional and spiritual support beyond its clinic at the Anchorage Native Primary Care Center.

Fresh Start Expansion — The Section of Chronic Disease Prevention and Health Promotion within the State of Alaska Department of Health, Division of Public Health, proposes to increase access to chronic disease prevention and management programs for Alaskans statewide.

This service reduces the risks associated with placing young adults in mixed-age adult settings, where exposure to older, higher-acuity populations can negatively impact safety, engagement, and recovery. South Peninsula Hospital

Expansion and Improvement of Rehabilitation Services to Support Continuum of Care Close to Home — South Peninsula Hospital proposes to renovate and equip existing hospital space to open the first cardiopulmonary rehabilitation program on the southern Kenai Peninsula. Residents recovering from heart attacks/surgery and chronic lung disease currently must travel 150 miles round trip twice weekly for rehabilitation services, a prohibitive barrier for many. As the region’s population ages, the need for this service has increased. This project brings medically supervised exercise, education, and recovery support close to home for about 17,000 rural Alaskans.

Mental Health Supports for Early Childhood — CCS Early Learning will establish a Behavioral Health Associate training hub — using Head Start classrooms as a real-world learning environment — and an early childhood mental health clinic for children ages 0–5. In 2023–24, 44% of CCS-enrolled children qualified based on foster care or homelessness — 7 times the national Head Start average — documenting acute need for expanded behavioral health services in Alaska's Matanuska-Susitna Borough.

New EKG Monitors — The City of Bethel proposes to purchase three new state-of-the-art EKG monitors/defibrillators, now called cardiac monitors/defibrillators, to replace three ten-year old cardiac monitor/defibrillators stationed in three ambulances used by emergency medical service responders. The current equipment is at the end of its useful life, produces slower diagnostic results, is difficult to find parts for and repair, and has experienced heavy use due to Bethel's high call volume. Bethel EMS personnel serve Bethel residents and visitors 24 hours a day, 365 days a year.

Increase access to low-cost medications for Interior Community Health Center patients who are living in poverty. — Interior Community Health Center (ICHC), a Federally Qualified Health Center (FQHC), located in Fairbanks, within the Fairbanks North Star Borough (FNSB), a hub to the surrounding Denali Borough, Southeast Fairbanks Census Area and Yukon-Koyukuk Census Area. In 2025, ICHC saw 2,361 people living at or below 200% of poverty, with a significant number unable to fill their prescriptions due to the cost. ICHC proposes to address this barrier by expanding its 340B Drug Pricing Program to give patients access to discounted medications.

Aleknagik Rural Health Access Strategic Plan — The City of Aleknagik proposes a 12-month strategic planning effort to address healthcare access barriers for this remote community of approximately 200 residents on the shores of Lake Aleknagik in Southwest Alaska. The plan addresses three areas: a sustainable rural workforce strategy including recruitment pathways and clinician housing options, an operational review and capital improvement plan for the municipally owned clinic, and transportation and connectivity improvements to strengthen access to higher levels of care. Deliverables include a costed, 5-year phased implementation roadmap adopted by City resolution.

Southeast Alaska Cancer Care Support Network — Cancer Connection will create a Southeast Alaska Cancer Support Network, uniting nonprofit organizations serving rural communities across the region. Without this intervention, residents of Girdwood, Hope, Whittier, Bird Creek, and Indian would face 40–90 mile trips to Anchorage for prescription medications.

The project responds to a shortage of trained health care workers in Galena, the Yukon-Koyukuk region, and the rural and remote communities served by the district's statewide residential school, where local elder care, home health, and clinics struggle to recruit and keep staff.

Interior Community Health Center

Improve blood pressure control in 50% of the 320 high needs patients who have uncontrolled hypertension by June 30, 2027. — Interior Community Health Center (ICHC), a Federally Qualified Health Center (FQHC), located in Fairbanks, Alaska within the Fairbanks North Star Borough (a hub to the Denali Borough, Southeast Fairbanks and Yukon-Koyukuk Census Areas) proposes to improve blood pressure control in 50% (160 of 320)of its patients diagnosed with uncontrolled hypertension. ICHC proposes to do this by using a remote patient monitoring (RPM) nursing service linked to the patient's home blood pressure cuff and their electronic health record (EHR), utilizing an individualized health promotion coaching intervention.

Railbelt Mental Health & Addictions

Building Capacity for Certified Community Behavioral Health Clinic (CCBHC) Status — Railbelt Mental Health & Addictions proposes a planning and readiness phase toward achieving Certified Community Behavioral Health Clinic (CCBHC) designation in a frontier region spanning 180 road miles of the Parks Highway, serving communities of 100–800 residents approximately 50–150 miles from Fairbanks and Wasilla. Planning activities include conducting a needs assessment, determining CCBHC certification requirements, engaging stakeholders, and building the administrative and operational infrastructure necessary for a full CCBHC application. CCBHC designation would enable same-day assessments, 24/7 crisis services, and comprehensive behavioral health care regardless of ability to pay.

Aleutian Pribilof Islands Association

Strengthening APIA Rural Diagnostic Capacity through Laboratory Expansion — The Aleutian Pribilof Islands Association (APIA) proposes to expand laboratory capability at all four Primary Care Services clinics in the remote Aleutian and Pribilof Islands. This project will install a Piccolo Chemistry Analyzer at Nikolski Health Center and deploy troponin-capable point-of-care diagnostic systems at all sites, including the Oonalaska Wellness Center hub. These upgrades will enable timely onsite testing, reduce delays in critical care (especially cardiac events), minimize costly patient travel and medevacs, and improve health outcomes for rural Unangax communities.

City and Borough of Juneau

Community Health Data and Billing Infrastructure — The City and Borough of Juneau's CCFR Community Health Division proposes to hire a contractor to build billing and data infrastructure within the Imagetrend platform for its three community health teams—Mobile Integrated Health, Mobile Crisis, and Sleep-off/Sobering Center—which currently conduct nearly 5,000 annual contacts with no reimbursement or billing structure. The contractor will develop Medicaid/Medicare billing systems and automated reporting dashboards to support program sustainability and quality improvement. The exchange builds and maintains advanced clinical skills that low patient volume and geographic isolation make difficult to sustain, strengthening the emergency medical workforce that serves Borough residents and surrounding rural communities.

Doppler Ultrasound Upgrade — Norton Sound Health Corporation proposes to deploy sixty-five dual-probe handheld Dopplers with ruggedized cases to community health aides and advanced practice providers across remote clinics, addressing a critical regional gap in standardized diagnostic infrastructure needed to monitor maternal-fetal health and evaluate vulnerable elders and diabetic patients for vascular perfusion and peripheral deep vein blood clot risks.

Strengthening Rural Dental Care with Advanced 3D Imaging — Chugachmiut proposes to implement advanced 3D dental imaging services at the Chugachmiut Regional Health Center in Seward to address limited access to diagnostic imaging for rural residents of the Chugach Region. Patients currently travel outside the region for cone beam computed tomography (CBCT) imaging needed for complex dental diagnosis and treatment planning. Construction of New Exam Room — Optical Aleutians proposes the creation of a dedicated optical exam room to replace the borrowed dentist office currently in use, as it poses multiple issues regarding ADA compliance, sanitation, consistency of access, and ability to service needs of residents with chronic issues. This will be built in Unalaska, which serves as a healthcare hub for the rest of the western Aleutians.

Point of Care Ultrasound & PACS System — Arete Family Care, LLC proposes to acquire a diagnostic ultrasound system and integrated PACS to establish on-site imaging capability at our Anchorage primary care clinic, addressing the lack of accessible, affordable diagnostic imaging for uninsured, underinsured, Medicare, and Tricare patients who face significant delays, transportation barriers, and referral dropout when referred to external facilities for routine abdominal, pelvic, thyroid, and vascular studies.

The planning effort will focus on creating a coordinated, billable home care model that utilizes Community Health Aides, nurses, telehealth, and remote monitoring devices to support homebound and chronically ill patients across villages with limited clinic access. The goal is a sustainable, evidence-based home care framework that reduces avoidable hospitalizations and supports patients unable to travel to clinic settings.

Nenana Volunteer Fire/EMS Department

Nenana Mobile Crisis Team — The Nenana Volunteer Fire/EMS Department proposes a 6-month planning phase to design and operationalize a Mobile Crisis Team (MCT), given that behavioral health calls currently account for approximately 33% of all emergency responses in this community without a structured crisis response model or specialized training. Planning activities will develop operational policies, cross-agency coordination agreements with behavioral health providers and schools, workforce and training strategies, potential Medicaid billing pathways, and a phased implementation roadmap. The goal is a coordinated, treat-in-place crisis response model that reduces unnecessary emergency department use and out-of-community placements.

Denali Insight LLC

Nenana Community Health & Wellness Strategic Planning Initiative — Denali Insight Consulting proposes a community health and wellness needs assessment and strategic planning initiative for Nenana, a rural Interior Alaska community of approximately 350 residents that serves as a regional service hub but lacks a formal health committee or shared strategic framework. Hepatitis C Confirmation Test Machine — The Interior AIDS Association proposes to purchase a medical testing device that will provide a confirmed Hepatitis C diagnosis. The problem that this machine solves is that Hepatitis C positive test confirmations are expensive and hard to access in the Fairbanks North Star Borough, especially for people we serve who have active SUD and who may not have a primary care provider or insurance.

Ptarmigan Pediatrics LLC

Pediatric Specialty Access Planning Initiative - Expanded Telemedicine Capabilities (PSAPI) — Ptarmigan Pediatrics proposes a one-year, $20,000 planning initiative to evaluate the feasibility of a multidisciplinary telemedicine framework for complex pediatric evaluations—including FASD, neuropsychology, and speech therapy—delivered from a Wasilla hub to rural Alaska communities through 'sister site' partnerships with remote clinics. A Nurse Practitioner coordinator will conduct 3–4 rural site visits to assess broadband connectivity, clinical space, and staffing capacity, and develop MOUs and workflow designs for hybrid telemedicine delivery. Many residents of the Bristol Bay Area have un- or undermanaged hearing loss, which makes it difficult to communicate with caregivers and family members in a clinical setting. BBAHC will provide low cost, high quality, and durable Assistive Listening Devices to village-based clinicians and Community Health Aides to use when interacting with patients in one-on-one settings.
